Essential Duties:
Promotes a welcoming, positive customer service culture which encourages quality patient outcomes and promotes staff retention Interview RSC patients within 24 hours of admission Assist patient to identify potential family and community supports available on their return to the community Provide information and support for patient's family and other significant relationships, with a goal of creating understanding of SUD/MAT services, with new knowledge utilized to support the patient post RSC or other treatment programs To provide individualized, ongoing guidance, coaching and support To provide training in the use of personal and community resources Assist in developing formal and informal community supports Assist the person served increasing social support networks of relatives, friends and/or significant others To offer encouragement in times of crisis and stress to eliminate potential barriers to one's ongoing recovery To advocate on behalf of persons with behavioral health problems to protect the client's rights and to assist in reducing associated stigma To participate in required staff meetings, supervisions and needed trainings as deemed by Clinical Program Manager To perform other related duties as assigned Schedule:
